Crossing at San Ysidro for a medical appointment
The practical detail nobody publishes: documents, timing, what the queue is actually like, and how to plan a same-day appointment from San Diego.

For patients living in southern California, the distance to a Tijuana clinic is short. The uncertainty is not about distance — it is about the border, and most clinic websites skip past it in a sentence.
Documents
To re-enter the United States you need a passport, a passport card, or another approved travel document. A driving licence alone is not sufficient, and this catches people out every week.
Timing
Southbound crossing is generally quick. Northbound is the one to plan around, and waits vary enormously by time of day and day of week. Late afternoon and Sunday evening are consistently the worst; mid-morning on a weekday is usually the best. Wait times are published live by US Customs and Border Protection.
Walking or driving
Many patients park on the US side and cross on foot, then take arranged transport to the clinic. Pedestrian queues northbound are typically shorter than vehicle queues, and you avoid taking a US-plated car into a city you do not know.
If you have mobility limitations — which, for an orthopedic patient, is not unusual — tell your coordinator. The walk from the pedestrian crossing is longer than people expect.
Planning the day
Treatment is outpatient. Most patients crossing from San Diego are back on the US side the same afternoon. Do not schedule anything important for the evening of your treatment day — not because you will be unwell, but because border timing is the one part of the plan nobody controls.
On safety
It is a fair question and deserves a direct answer rather than reassurance. Tijuana is a large city with the variation any large city has. The clinic district is residential, the route from the border is straightforward, and patients using our transfer are accompanied throughout. The sensible advice is the same as for anywhere unfamiliar: arrange transport in advance rather than improvising on arrival.
